There are any number of reasons for upper back and neck pain, and it is fairly common.  It may come on suddenly and painfully, or may progress over time as a result of poor posture and other reasons.  When you have upper back and neck pain, it can keep you from thinking clearly, concentrating, and being motivated.   Upper back and neck pain causes emotional stress, which worsens the pain.  We will be discussing some of the more common injuries.

There are two different types of neck pain, acute and chronic.  Neck pain that is related to soft tissue, such as muscles, ligaments, or tendons, is referred to as acute.  Disc herniation can cause acute neck pain.  Chronic pain may be hard to determine and persists for longer than 3 months.  It may get worse during certain activities and if your posture is poor.  Poor posture causes body mechanics to be out of line.

If there is neck pain along with headache, nausea or fever there is a possibility of meningitis, an infection in the membranes surrounding the brain.  It can also be a symptom of a brain hemorrhage.  You should call your doctor if your neck is stiff and you can’t touch your chin to your chest.

Your neck and upper back work together when you move your head, shoulders, and arms.  The joints are smaller than the lower back so they don’t handle pain and stress very well.  The muscles supporting the joints are smaller and can tighten more quickly than the lower back muscles, causing pain.

The cervical spine is the neck portion of the spine.  A non-radiating pain is called Cervicalgia.  Your neck muscles work to hold up your head, which is actually quite heavy, and weighs approximately 10 pounds.  There can be pain from tightening of the muscles or pain as a result of sudden movements.  If your head is not held over the neck correctly and is in a forward position there is strain on the muscles.  This will cause aching in the neck, and sharp pain when turning your head.  There is a tight feeling in the neck and upper back.  Your neck and back muscles may be tender to the touch.  Stiff neck and headaches are common as well.

If you think a muscle may be torn from a sudden movement, you will want to ice the area and take anti inflammatories.  If it is severe, you can wear a neck collar to help muscles rest.  If the pain lasts for more than 5 days, try applying heat and it’s a good idea to call your doctor.



A pain that radiates to other areas indicates that nerves may be compressed.  This can be the result of bony deposits, damage to a disc, or tightened muscles.  The pain may radiate down the arm from the shoulder to the fingers.  It could radiate up into the head and cause headaches or dizziness.  With these symptoms it’s best to contact your doctor so the cause of the pain can be determined.

Tightness in your upper back and neck can cause pain, because the tightness of the muscles doesn’t allow as much blood flow so energy and nutrients can make their way to the muscles.  If the muscles aren’t healthy, they will tighten even more.  Tightness in your muscles comes from small tears in the muscles from overuse.  The muscles protect themselves by tightening.

When you are working out, it is best to do stretching exercises to avoid injury during your work out.  Make sure to do adequate stretching so your muscles don’t shorten.  If you have bad posture it can also cause muscle tightness in your upper back and neck.  A curvature in the spine caused by scoliosis can cause more strain on the muscles than they can handle.

A cervical posture syndrome, known as Kyphosis, occurs when shoulders are rounded and the chin points forward.  It is also called an upper crossed syndrome.  Your posterior and pectoral muscles will be tight and the upper back muscles and neck flexors are weakened.  These muscles cross, giving the syndrome its name.

Whiplash is caused by a rapid movement of the head, backwards and forwards.  This is most common in car accidents, but can be caused by sports as well.  Pain is caused by a ligament sprain of the Sternocleidomastoid, longus colli, and levator scapulae.  In severe cases there can be fractures of the vertebrae or nerve damage.  Pain from whiplash may not start until 24 to 48 hours after the injury.  Your neck’s range of movement is reduced, and you may experience headaches, dizziness, and blurred vision.  Depending on how bad the injury is the stiffness and pain may last from a day or two to weeks.

If you have other symptoms, such as numbness in your shoulders or arms, memory loss or a severe headache, it could mean a more serious injury such as a concussion.  It is best to consult your doctor or go to the emergency room if you suspect whiplash or more.



 Other than the causes discussed above, neck and upper back pain can be caused by stress, disease, wear, and tear.  Sleeping on your stomach and lifting objects improperly can stress the muscles and make them more vulnerable to injury.  Osteo arthritis, also known as Degenerative Disc Disease, occurs from natural wear and tear, forming bone spurs and inflaming joints.  Discs dry out causing shrinkage, loss of cushioning, and loss of flexibility.  This leads to stenosis or herniated disc.

In order to avoid neck and upper back pain re occurring, which is common, you should practice proper lifting, appropriate work outs, a healthy weight, relaxation, an ergonomic work space, and good posture.
